7.0 Health and Care. PRIORITIES. continued...

The major health service providers are committed to reducing the death rate from cancer in people under 75 by 20% by 2010, and reducing the gap between death rates the town and the more deprived commuities, including Dyke House/Jackson.

The under 18 conception rate (conception per 1000 population female 15-17) in Dyke House/Jackson was 107 (95-97 average), significantly greater than the Hartlepool rate of 75.5 which is greater than the national rate.

Key health partners have agreed a 15% reduction in the conception rate per 1000 population on 1998 baseline by 2004.

There are high levels of household members suffering from anxiety, problems with nerves, depression, isolation and loneiness. Dyke House/Jackson has a higher rate than in Hartlepool as a whole. There are high numbers of carers in Dyke House/Jackson.

KEY ACTIONS.

*Research into local needs, demand and potential supply of local health facilities in Dyke House/Jackson, and the identification of an organisation who can provide a local directory of available health services to help overcome access issues. *Local fitness and healthly lifestyle is to be explored by NRF Health Development worker in Dyke House/Jackson. *Health Development worker to maximise local opportunities. *Continue to promote the Heartbeat and Action on Smoking awards.

*Reducing the risk - smoking cessation, diet alcohol, exercise health development activities to be promoted by the local Health Development Worker. *Develop a HIV/Sexual Health Local Action Plan that will include Dyke House/Jackson as a priority area. *Continue to support the Dyke House school nurse. *Ensure an appropriate range of resources on sexual health issues is available to a number of key groups, particular those considered "at risk":- RESPONSIBLE ORGANISATION = Hartlepool PCT.

*Detecting cancer earlier - increasing public awareness and extending screening. *Improving cancer services in the community by faster access to treatment, equal access to services, support for people living with cancer:- RESPONSIBLE ORGANISATION = North Tees and Hartlepool NHS Trust.

*Further develop and deliver a SEx and Relationships Support package for Secondary Schools, including Dyke House School:- RESPONSIBLE ORGANISATION = Hartlepool Borough Council.
*Further develop links between the smoking cessation service and community:- RESPONSIBLE ORGANISATION = Teesside Smoking Cessation Service.
*Support the implementation of the Hartlepool Teenage Pregnancy Strategy at the neighbourhood level:- RESPONSIBLE ORGANISATION = Hartlepool Teenage Pregnancy Strategy Group.
*The development of integrated care services through the NRF funded Integrated Care Team should assist in providing support to those in need:- RESPONSIBLE ORGANISATION = NSF for Older People Local Implementation Team.
*Support for carers is provided through Hartlepool Carers, a voluntary organisation whose services will be promoted in the area:- RESPONSIBLE ORGANISATION = Hartlepool Carers.
*Current and planned activities under this and other theme areas should impact upon these issues. Within the neighbourhood Sure Start have built upon the NDC MIND project to provide mental health support locally:- RESPONSIBLE ORGANISATION = most of the partners mentioned.
