If you would like to make your own form you have to follow a few basic steps.
If you would like to make your own form you have to follow a few basic steps. The first thing you have to do is tell your computer that you are going to make a form by using this code:

<FORM METHOD="POST" ACTION="mailto:your e-mail address">

Now that the computer knows you are going to use a form it is going to look for a form style. I will show you five.

The first one is called a Text box, this allows visitors to type one line of text. You could use this box to ask visitors for there name, e-mail address, they website’s url, etc. You would use this code:

<INPUT TYPE="text" NAME="NAME" SIZE="30">
Here's what a Text box looks like:


The second one is called a text area box. It allows visitors to use as many words and sentences as they choose. A lot of people use it as a comment box. Here is the code for it:

<TEXTAREA NAME="NAME" ROWS=6 COLS=40> </TEXTAREA>

The third one I’m going to show you are called the Radio button. They only allow your visitors to choose one choice. Here is the code:

<INPUT TYPE="radio" NAME="BUTTON HEADING" VALUE="BUTTON NAME">

The fourth one is called a Checkbox. It allows your visitors to choose as many options as they like. Here is the code:

<INPUT TYPE="checkbox" NAME="NAME" VALUE="HPOPININS">

The fifth one is called the Pop Up-Box. Here is the code:

Finally you need a submit button to send to your e-mail address or a reset button to refresh the form. Here’s the code:

And for the last step you need to add this at the end of your form:
