LSK manages the 48 IS MDR program and provides information and guidance as required. On this page are links to the applicable Instructions, MDR Worksheet, and instructions for completing an MDR.
LSK serves as the focal point for MDRs. LSK�s major tasking in support of the MDR program is to function as the screening point. As the screening point, LSK has overall management responsibility and serves as the focal point for all submitting organization tasks. The screening point will interact with the originating activity, as required, to ensure the MDR is valid, complete, accurate, and properly addressed; assign report control numbers (RCN); ensure proper marking and handling of exhibits; transmit outstanding reports; and act as the focal point for communications with the contact/action point (as applicable).
When a part is identified as being bad from supply or manufacturer, Workcenters will fill out: A) MDR worksheet B) 350 Tag C) Fill out two each DD Forms 1575 (Brown Tag) and DD Form 2332 (Exhibit Cream Tag) D) Bring the MDR paperwork, the 350 tag and a copy of the Serviceable tag that came with the part to LSK. LSK will send the deficiency report. E) Upon notification from LSK, attach the Brown Tags, 350 tag and Exhibit Cream tags to the exhibit and transport the item to supply.
