BEHAVIOR CARDS
I. Teachers are responsible for
calculating their 7th period student's behavior card.

Make sure blanks are properly filled in. (name, date,
and ID#).

When a student is absent, write ABSENT and the DATE
under the correct day.

If a student's absence is UN-EXCUSED as per admit to
class slip, enter "0" for the days total.

When a student is absent only part of the day and has an
excuse for those periods, be sure you make those adjustments when
figuring Total Possible Points for the week (total points that will be
used to divide by). Example: John was absent four periods for the
day, so you divide the total points earned by 124 instead of 140.

It is the STUDENT'S RESPONSIBILITY to bring in an excuse
for his/her absence(s) on the same day he/she returns to school in order
to earn points for that day and to be considered to participate in an
activity. Excuses brought after the 2 day deadline will be
considered UN-EXCUSED as far as activity participation is concerned.

When a student is absent, the 1st period teacher will
turn in the student's behavior card to the secretary. The office
will return the card to the student when the student goes to the office
for his/her Admit to Class slip (before 9:15 AM). If the absence
occurs on a Wednesday the secretary will keep his/her care but the
student must still go to the office for an admission slip.

Earned fill in the total
points earned during a one week period.

Bonus fill in the bonus
points earned on the back of the behavior card.

Fines fill in the total
number of points a student was fined on the back of the behavior card.

Earned points + Bonuses - Fines = Total Behavior Points
for the week.

Total Behavior points/Total Possible Points = Behavior %
for the week. Explanation: Student may earn 28 points per day:
28 points X 5 days = 140 points
28 points X 4 days = 112 points

When a student has earned enough points to attend an
activity and he attends the activity, give the student 4 points for each
period he/she is at the activity.

Students must maintain a 90% average on the behavior
card for a certain number of weeks to attend an upcoming activity.
II. Contracted Status
Occasionally a student must be placed upon a contract when
their inappropriate behavior warrants such contract. Below are the
procedures for placing a student on a contract.

Criteria:
a. Continued problem behavior not responding to or not covered by
the behavior management system.
b. Placement by the principal or the principal's designee.
c. Placement by the Academic Team.

Students are issued an appropriate care with a yellow
line through the day(s) section covering the time of the contract.

Instructional arrangements options:
a. The student will be placed on a contract outlining specific
behaviors and consequences. Special arrangements (such as daily
meetings with staff member) will be delineated.
b. The contract may be developed by the principal, his designee,
or the Academic Team.
c. The student may be assigned to restricted meals.
d. The student may be assigned to a special class. This
class would emphasize social skills and/or problem solving. The
assignment time would be for the length of the contract.
