1)    When an employee does a good job, I tell him/her so and encourage him/her to do that   behavior again and again.
2)    If I encourage an employee enough, eventually they perform the behavior without me needing to constantly offer my support.
3)    I provide timely praise/rewards for good behavior.
4)    My rewards are strictly dependent on specific behaviors. 
5)    My employees seem to reinforce each other, maybe due to my example.
6)    The type of reinforcement I demonstrate is appropriate to the behavior.
7)    I have implemented feedback tools in the work environment, i.e. e-mail, performance appraisals, and group recognition events.
8)    I keep the contingencies of rewards consistent to specified behaviors.
9)    The rules for earning rewards are clear.
10)  The rules are fair and do not pick out "favorites".

Scoring:
If you scored between 10-23 on this scale your feedback components have much room for improvement.  If you scored between 24-36 your feedback components need some work.  If you scored 37-50 you effectively use feedback components!  But, there is always room for improvement!
