McCoy se inyecta por error una jeringa completa de cordazina, y en pleno delirio desciende al planeta alrededor del cual está orbitando la Enterprise. Kirk y Spock, siguiéndolo, descubren la Puerta del Tiempo, una máquina-criatura capaz de hacer viajar en el tiempo. McCoy, enloquecido, entra por la Puerta, cambiando el curso de la historia.

Notas:

• Las diferencias entre la historia original de Harlan Ellison (que le hizo ganar el Writer's Guild of America para la temporada 1967-1968) y la versión final del episodio, adaptada por Gene Roddenberry, han sido siempre una vexata quæstio, a pesar de que este episodio es considerado con mucho el mejor de la serie. El motivo de los cambios aportados por Roddenberry es que Ellison había escrito este episodio cuando sólo habían sido realizados TOS: The Cage y TOS: Where No Man Has Gone Before, y por lo tanto las características del Universo Trek no habían sido definidas. Además la historia se centraba en el hecho de que era un drogadicto el que iba por error al pasado, lo que no gustaba a la producción y estaba en completa contradicción con los presupuestos del Universo Trek.
• La música de fondo que se escucha cuando Kirk y Edith Keeler pasean de la mano es original de los años 30 (una parte de Good Night Sweet Heart).  A partir de mediados de los años 80 las versiones home video tienen una melodía distinta, probablemente por motivos de derecho de autor. Sin embargo, aunque todas las versiones home video lleven en el estuche la leyenda "Some of the incidental music has been changed for this Home Video Version", algunos videocassettes llevan la melodía original. La versión DVD Zona 1 lleva la melodía original, a pesar de que el estuche lleve la misma leyenda de los videocassettes.

• En uno de los depósitos de basura de la ciudad se ve el símbolo que indica contenedor de material radiactivo.
