| Winter Art and Chocolate Festival �My Favorite Chocolate Recipe Contest� Rules: � This is a juried �Recipe Contest�. � Only one recipe per application and $5 entry fee. � Multiple recipe entries are allowed for $5 each. � Each contestant must cook, bake or prepare his or her own food goods. � No bakery items, store bought cakes, pastries or baked goods will be accepted. � Prepared ingredients such as Oreos, chocolate chips and candy bars may be included in the recipe. � Each entry must include the complete recipe with preparation instructions written neatly on one or more 3x5 card(s). � The contestant�s name must be written clearly on each recipe card. � The source of the recipe must be included on this entry form. Any entrant not complying with these or any other �My Favorite Chocolate Recipe Contest� rules may be disqualified from the contest. We reserve the right to disqualify any item deemed unacceptable by the staff of the Winter Art and Chocolate Festival.
