Civil Registry

Births, marriages, and deaths of Filipino citizens abroad must be reported and duly registered at the Philippine Consulates. The Philippine Consulate registers births, marriages celebrated in, and deaths of Filipino citizens in the State of Hawaii and the Territory of American Samoa. These are the requirements for the following civil registry reports:

I. Report of Birth

  1. Duly accomplished Report of Birth form
  2. Original birth certificate and two photocopies
  3. Copies of passports of parent/s
  4. Copy of US Passport
  5. Copy of the parent's marriage certificate. If parents are not married, then submit an Affidavit of Illegitimacy
  6. $25 Report of Birth fee

II. Report of Marriage

  1. Duly accomplished Report of Marriage form
  2. Original marriage certificate certificate and two photocopies
  3. Three photopies of passports of both parties
  4. Three 2x2 photos each of both parties
  5. $25 Report of Marriage fee

III. Report of Death

  1. Duly accomplished Report of Death form
  2. Original death certificate and two photocopies
  3. Copy of passports of deceased
  4. $25 Report of Death fee

IV. Consular Mortuary Certificate

Consular Mortuary Certificates are issued when remains or the cremated remains of a deceased are to be transported to the Philippines. The following are the requirements:

  1. Duly accomplished Report of Death form
  2. Certificate of Death issued by the State Department of Health
  3. Duly notarized certificate issued by the Mortuary Director that the body was properly embalmed or cremated.
  4. Certificate of No Contagious Disease issued by the State Department of Health.
  5. Burial transit permit
