I forget the name of Aeris' ultimate weapon, but you find it in one of the doors with the clock in the Temple of the Ancients.
Great Gospel - Get the mythril from the sleeping man near Junon, you should get it if you've fought a number of times with 11 at the end. Then go to the blacksmith near Gongaga town. Choose the SMALL chest and you'll get it. If you choose the big chest, you'll get a gold armlet.
Limited Moon - Go to Cosmo Canyon and visit Bugenhagen in the third disc. He will give it to you after leaving on his journey.
Cosmo Memory - Defeat Lost Number in the Shinra Mansion, and you will get the key, and Cosmo Memory.
Conformer - In the Gelnika, search the room to the side when you walk in.
All Creation - Defeat Godo. Easier said than done.
