The Class 1 Medical Form is a simple form listing emergency contact people and any allergies, etc.
Anyone sailing with Ship 1000 should provide this form to Catherine.
This form should be provided by all non-member youth sailing with Ship 1000.
(Requires parent signature for sailors younger than 18.)
MEMBERSHIP

This form should be completed by any youth (age 14 or older) who would like to join Ship 1000 as a Member and submitted to Catherine with the annual $10 dues.
(Requires parent signature.)
CLASS 3
CLASS 1
The Class 3 Medical Form is a required for all Venture Scout groups due to the high-adventure nature of our activities.
(This form should be submitted by all members, and requires a physical / doctor's signature.)
The Class 1 Medical Form is a simple form listing emergency contact people and any allergies, etc.
Members should submit this form to hold until Class 3 can be signed by doctor.
