On your appointment date please be punctual and bring
a Certificate of Disability from your primary Physician.
Your certificate must include an HIV diagnosis, and
date of disability. Forms are available if requested.
Bring picture ID of yourself (drivers license, passport, etc)
Check cashing cards or health club ID cards cannot be used.
