Be sure all information is accurate as this will be what the coach keeps on hand to pull out in case of an emergency.
Must be signed and turned into coach by the first day of tryouts.
Must be signed by the cheerleader and parents then turned into the coach by the first day of tryouts.
Parents must sign and return to me before I can publish any group or individiual photos to the web site. Your childs photo will be blurred out in any group photos.
Must be signed by the parents before you come to tryouts..

Your uniform and equiptment numbers will be listed on this sheet. The cheerleader and parents need to confirm the assigned uniform numbers, sign and return this sheet to the coach by the first week of practice
