| Maternal Family History |
| All of my mothers relatives from her her parents on back passed away by mid fifties from heart disease. They have a history of premature death, obesity, depression and cancer. |

| Paternal Family History-don't know much about my Fathers side, we didn't have contact with them while I was growing up. They did however, have a history of heart disease and cancer. |

| Father-Morbidly obese, BMI 50.6, he had health problems, had his gallbladder removed, had his knee operated on because of stress on the joint, hypertension, hepatitis, post surgery phlebitis, died at the age of 46 from metastisized colorectal cancer. |
