The Hi-Lift jack. 
definitely great off road, as it can be used as a jack and a winch.  Very tough.  Summit Racing sells them, but you might find them cheaper elsewhere.

The tonneau cover. 
A great investment.  Keeps gear dry, looks good, and is very tough.  It's a TekStyle by Design Automotive group., I got it from Summit Racing, and it was only around $200.  They have applications for every truck.  Well worth the money.

Fullsize spare.
I learned the hardway- donut spares suck to drive on.  If you've got room, get a fullsize.  New ones are expensive, but at least go get a used one.  Mine was only $40 with a used rim to match.
