Application/Assessment Process
1. Call an In Unison representative at (909) 931-4844 to request services.
2. Call your Regional Center Case Manager and ask for your personal history and information to be sent to In Unison to begin services. You can also ask a responsible family member or significant other to request this information for you.
3. We then schedule an interview with you and set up a time and place to meet you. (Interviews wit family, friends, day/work program or Regional Center representative or others will be arranged as needed.)
4. In Unison will develop a preliminary Personal Sucess Plan and meet with you to discuss it, make changes and have you sign your approved plan. We then deliver your plan to your Regional Center Case Worker.
5. The Regional Center Worker will then get approval for the plan and give us a date that services can begin. In Unison will then call you and set up a schedule to start-It's that Easy!
