Last time around our heroes and villains were about to face the invasion of the Skrulls.
Two years have passed since.
The Skrull attack devastated the Earth. But the combined efforts of both the X-Corps and the Resistance were able to drive them away, not without casualties though. Psylocke disspeared shortly after the Skrull invasion was over.
Onslaught's hold over the Earth began to fade.
The Resistancers took over Europe, with the help of the Scarlet Witch, who betrayed Onslaught. She was elected ruler after Magneto's death (Magneto was actually Jospeh, the real Magneto however remains missing since the Fall of Avalon). Afterwards Dr Doom was sent to the Resistance Island to help them find a way to separate Xavier from Onslaught.
Shortly after things began to go hectic. Jean's powers became uncontrollable. She was ruthless, and even killed a foe in battle. But it would't last long. Psylocke returned with Longshot and Dazzler. She then found Wolverine, Rogue and Colossus, who together with Storm and Havok formed the Order of the Phoenix. With their new powers given to them by the Crimson Dawn, they were able to remove the Phoenix Force from the body of Jean Grey, making it impossible for it to return to our solar system.
The fight was long, Psylocke vanished after it and heven't been seen since. Onslaught lost his greatest ally.
After several fights Dr Doom, with the help of Doctor Worm and Forge, was able to create a device to separate Xavier from Onslaught. But that wasn't just it.
Xavier, Franklin Richards and Nate Grey were liberated, but none retained their powers.
Onslaught, however, did. The fight was hard. The formerly mind-controlled heroes (like the FF, the Avengers, the Defenders, and most everyone else) fought against Onslaught and his few remaining loyal followers. Defeated Onslaught made a last, desperate act. He released all the imprisoned supervillains from all over the world and dissapeared, leaving the Earth to the Resistance.

A new World order had to be established. Captain America insisted in restoring the Earth to pre-Onslaught status. That was impossible. Chaos would have torn the planet apart. A new Ruler had to be chosen. Someone firm, yet kind with his subjects. In the end there wasn't any other choice. Victor Von Doom finally got his life-long dream, he became the ruler of the Earth. His first official act was to marry the woman he loved. And Storm became the Baroness Von Doom.
The former Resistance now serve Emperor Doom, and protect the Earth from all the villains that roam it.

But in the shadows of the recently created government... Onslaught awaits for a chance to return to his seat of power...

Clearing things up a bit...

first 5 years: Onslaught in control
then Skrull War: lasted 6 months.
Then Order of the Phoenix affair: A couple of months after
Onslaught's fall: 3 months later
Anarchy and chaos: As Onslaught was defeated he released the villains. This period lasted 6 months. CAp wanted to make the world like it was pre-Ons.
Doom's rule: 6 months ago. Things have stabilized a little but a lot of villains are still on the loose, concentrating the main attention of the heroes.

A little girl, Kaie Power has arrived on Earth looking for heroes.
Magneto and Apocalypse prepare to make their move.

Jean has awakened from her year long slumber.
