[In this section of your Affidavit, briefly list the details of your story: Include things like:
What false allegations were lodged against you;
The date your child(ren) was taken and how long you have been or were deprived of your children;
Describe the circumstances your child(ren) were seized, if the seizure (taking of your children) was unreasonable, explain how and why it was unreasonable;
Describe how your rights and your children's rights were violated:
Describe how you and your were treated by CPS;
Describe the effects of all this on your children and what happened to them in foster care;
Try to list a chronological list of events (Dates that things happened in your case, seizure of children, initial hearing, adjudication hearing, dispositional hearing, etc) and the outcomes;
List the names and titles of social workers and others involved in your case;
Include any other pertinant information.
Remember, try to be BRIEF and just list the facts.

List your name, mailing address on each page of your evidence book and number the pages. For example, if your evidence is three pages, list page "1 of 3", "2 of 3", etc.
