For all of you that has a story to tell, here is your chance to make your story part of our NATIONAL CONGRESSIONAL RECORD. All stories will be read.


CONGRESSIONAL COORDINATOR HAS REQUESTED THAT ALL SPEAKERS SUBMIT EVIDENCE IN A STANDARDIZED FORMAT AS FOLLOWS. (SUGGESTION: Do the best you can and get your evidence book mailed.)
Victim Witnesses:
Victim Witnesses. Individuals and families, victims of the Child Welfare System and Courts who have evidence of crimes (including crimes of omission such as police or CPS refusing to investigate) committed by government employees and vendors (such as GALs, psychologists, expert witnesses, visitation supervisors, court appointed attorneys, foster/group homes) including perjury, falsified documents, child endangerment (including sexual abuse and homicide, courts placing children with abusers, etc.), coerced testimony, extortion, etc . Each speaker will have ten minutes. Please submit your documents in a black three-ring binder (with clear plastic insertion cover on front and spine) that includes in this order:

1) a cover sheet with your name, relationship to the child(ren), contact information, your county of residence (example: Frankfort County), your congressional district number (example: 61st Congressional District of Indiana), your state legislative district number (example: 42nd Assembly District of Indiana), and date ;
2) a typed, double-spaced copy of your spoken remarks; (or your written speech)
3) your brief resume;
4) a chronological timeline of events in your case from the first CPS contact to the current status of your case;
5) list showing names and titles of all persons involved in your case , including family members [indicate relationship to child(ren)], names and titles of social workers and their supervisors and managers (indicate any degrees and licenses, if unlicensed, so state), county counsels and other attorneys for both sides (include state bar numbers), judges (including commissioners and referees, if no oath of office letter available, so state), foster parents, name of group home, name of non-profit corporations, visitation supervisors, law enforcement officers, medical professonals/institutions, note any official complaints filed against individuals/corporations listed; 6) a brief, one- to three-page descriptive synopsis of your case, double spaced;
7) a table of contents for succeeding documents of evidence in your case (explain briefly what each document proves);
8) evidence;
9) media articles about your case (electronic media appearances can be listed, identify station, name of show/newscast/documentary, and dates of appearances) .

Please do not exceed 100 pages. You may be asked to submit additional evidence at a future date. If you are submitting legal cases as evidence only the first page with the case number and court seal and the accompanying minute order are necessary at this time. Beginning with your remarks
(2), please identify each page in the upper-right corner with your name (Last name, first initial) and page number (1 of 100). All pages must be on white paper (to facilitate clear copies). Outside identification of binder -- Please insert a copy of your cover sheet on the outside front of the binder. On the spine of the binder please insert a white label showing your last name, first initial, relationship to child(ren) and state of residence (WV).

Expert Witnesses:
Expert Witnesses. Authors, attorneys, medical professionals, academics, other professionals, advocates and heads of organizations, will need a written speech that would take 10 minutes or less if actually spoken. Please submit your documents in a white three-ring binder (with clear plastic insertion cover on front and spine) which includes in this order: 1) a cover sheet showing your name, title, contact information and date (March 13, 2004); 2) a double-spaced copy of your spoken remarks; 3) your resume/curriculum vita; 4) a table of contents for succeeding documents; 5) documents you deem important to apprise members of the United States Senate and the House of Representatives. Please do not exceed 100 pages. You may be asked to submit additional evidence at a future date. Beginning with your remarks (2), please identify each page in the upper-right corner with your name (Last name, first initial) and page number (1 of 100). All pages must be on white paper (to facilitate clear copies). Outside identification of binder -- Please insert a copy of your cover sheet on the outside front of the binder. On the spine of the binder please insert a white label showing your last name, first initial, title and state of residence (example: CA).
