Jan. 1.  Lab monitoring was done on Nov. 30, 1994, all within acceptable limits. Recommendation: to continue observing Danny�s Rapid cycling bipolar mood cycle.
Jan. 18. The Geneva Centre Workshop. �Managing Rage Behaviour in Autism and PDD� by Dr. Huggins.

Feb. 2.  KP staff is concerned by his need for constant movement. Side effects of the medication?  Dr. Huggins considered them part of his �idiosyncratic behaviour� and advice to report if �behaviours escalate�.
Feb. 17.  Lab monitoring �all within acceptable limits. Dr. Huggins says, quote: �Should they (idiosyncratic  behaviours) escalate or fail to resolve then we should remove his medications�.

March 7.  WIC visit for suspected infection. Rx: Otitis media. Given Amoxil.

April 5.  Congested. Head banging, suspected ear/throat infection. WIC Rx (intelligible).

May 5.  Head banging, possible ear/throat infection. WIC Rx: (intelligible).
May 9   (Incident at Saturday program taken to Hospital got 3 stitches on knee, might be related to Ativan consumption) Stitches in knee removed
May 16. WIC visit. Danny says �sick� is crying. There is slight SIB. Not eating much.
Given Tylenol.

June 6.  Hit his head against a window suffers a cut over his left eye. ER York Central Hospital. Given 4 stitches after being sedated with 2 mg Ativan, 2x 5 mg of Haldol.
Dr. Huggins recommends ear tubes.
June 14. Dr Huggins thinks Danny suffers from Eustachian Tube Dysfunction with earwax build up.

July 19.  Clonidine if reinstating Valproic Acid does not help. Danny�s mood cycles are unpredictable and the combination of Tegretol and Propranolol is not effective in controlling this mood pattern.

Aug. 9.  Reporting SIB episode. Hitting his head with utensil and hand, 5:30 p.m. In the same day at 8:00 p.m. SIB episode: biting his shirt, rocking, singing loud, head banging, slapping and hitting his body. Aggressing staff.  2x given Ativan.
Aug. 29.  Danny is seeing by an Ear, Nose, Throat Surgeon Dr. Jerry B. Naiberg. He recommends bilateral myringotomy and tube insertion. (Family opposes).
