INDEX FOR THE YEAR 1994
June 23 Annual Physical Examination. Danny is on Propranolol 120 mg/day and
Tegretol 600 mg/per day. Flushing his ears to remove wax build up to prevent
infections.
June 24 Danny�s worker asks Dr. Huggins for Danny�s �own prescription
of Ativan�.
(It is not written the time of the day for this request)
June 24 At about 2:30pm Danny banged his head through back window of
van. Nose bleed after hitting head. Taken to York Central Hospital.
July 3. Lab monitoring all within acceptable levels.
Sept. 8 Seems to be favoring a sore throat. Losing his
voice. Throat infection? WIC visit more Rx Amoxil
Oct. 21 Cerumol? Antibiotics in ear drops?
Nov. 3. Head banging frequently for the past 3 days. Suspected sore
throat. Given Amoxil at Walk in Clinic (WIC).
Nov. 4. Too many times Danny is given antibiotics. (It is my objection).
Dr. Huggins wants to see if he is treated for a middles ear infection (Otitis
media), an external ear infection (Otitis externa) or both. Still problems
with earwax build up.
Nov. 11. Temperature of 102 o. Currently on Amoxillin for suspected
ear infection.
Nov. 15 Ears normal.
Nov. 20. Worker report an increase of SIBS (headbanging) and hyperactivity
since ear infection. (A reaction to Amoxil?) Concerned about his head banging.
Dr. Huggins increases Propranolol to140 mg and to continue recording his
pulse rate.
Nov. 22. Head banging, abrasions and bruises. WIC Doctor prescribed
Auralgan.
Nov. 25. Three SIB with head banging in one week, workers wonder if
it could be toxicity from Carbamazepine, by now 700 mg? Dr. Huggins says,
quote: �His observed SIBS, etc. must be of �Central� origin�.
Nov. 25 Head bangs, crying, suspected ear/throat infection. WIC Doctor
recommends checking drug level in blood.
