| New-members � New-members must complete the following: Membership Form, Health History Form (required by ACSM protocols and standards), Informed Consent/Release of Liability Form, a scheduled FitCheck, and Orientation session. >>>> FOR MORE ABOUT THESE SERVICES>>>>>> Previous Members � Previous members must complete the following: Membership Form, and possibly, an updated Health History form, and an Informed Consent/Release of Liability, if more than 6 months since last current, or changes exist. � Members are responsible for reporting any changes with their Health History Form, i.e. address, employment, medical, or medicine. �A FitCheck is recommended if it has been more than 6 months since being a member. � The $5 administration fee may apply, if renewing within 6 months of your last membership. Current Members � Current members are responsible for reporting any changes in their Health History Form. Membership Fees � Individual members can join for a 3 ($54), 6 ($108), or 12 ($216) month period. Buy 12, and get an extra month added, free. � Corporate members must join for 6 ($90) or 12 ($180) month periods. See your rep for details. � Reimbursements and/or transfers are not allowed. Payment Procedures � Individual payment must accompany completed forms. � Payment must be paid by check (ID req.), bank draft, or money order; credit cards are not accepted. � Corporate payments must be made to your company�s fitness center representative (designate a representative) � for details on payment procedures. The representative is responsible for turning in a (1) check for the full amount, of all members, to the fitness center, accompanied by the membership forms, one week prior to the start date of the membership. To maintain privacy, the Health History and Informed /Release of Liability Forms are required and should be submitted directly to fitness center staff prior to club usage. � Make check or money order payable to: �The Manufacturers Life Insurance Co. USA.� � A $25 service charge will be imposed for all checks returned due to insufficient funds. It is recommended that you pay by money order or cashiers check. Corporate Membership Rules � To set up a corporate membership, a minimum of 15 employees, joining at once, is required. � Each individual must be an employee of the participating corporation. � Memberships will commence on the first of any initiated month. � Corporate memberships are paid with one check, rather than collecting individual checks. (See Payment Procedures). The various existing member periods will be prorated accordingly. � New or returning members can take advantage of the discount rate, if he/she joins or renews at the beginning of the corporate membership cycle. Once the cycle begins, no additional members can be added to the corporate membership until the next cycle begins. Individuals can join, at any time, but will be charged the regular fees. Membership Rules � Members are expected to respect the clubs rules (see Rules) and be considerate to staff & others. Administration Fee � A $5.00 Reinstate-Administration Fee will be administered to all members who have not renewed his/her membership within 2 weeks of his/her expiration date. � This doc-processing fee will be added to the membership fee, before reactivating a member file that has been cancelled within 6 months. Cardkeys � A cardkey, for the fitness center, will be activated upon completion of forms. � If a tenant does not have a company cardkey, a fitness center cardkey will be issued. � A $20.00 refundable, fitness center, cardkey deposit is required. � The cardkey deposit can be made in the form of a check; credit cards not accepted. � The full deposit will be returned, via check requisition, when and if the cardkey is returned. � Members who received a fitness center cardkey before September 1, 1998 and did not pay a deposit will be charged $20.00 if cardkey is not returned after membership expires. EFFECTIVE as of JULY 1ST 2001 � Any current member, who REFERS A (first time) NEW MEMBER will be rewarded with 500 points into their accrued Club Rewards (Strive for Prize) account. The new member must name who should be rewarded for recruiting them. � CANCELLED memberships, as stated in the enrollment contract, are not entitled to any reimbursements. In addition, as a deterrent to cancellations, anyone who cancels will have a limited time to renew before losing all accrued reward points. Points are NOT transferable. � Any EXPIRED memberships will be considered Cancelled, if not renewed within 2 weeks, and subject to the administration fee. After one month as a cancelled member, half of your points will expire; after 2 months another half will perish; and after 3 months of a deactivated membership, all points will be lost. Points are meant to reward exercise adherence. � Membership FREEZING is not allowed, except under strict stipulations, (i.e. medical circumstances). Situations will be reviewed on an individual basis, and a maximum time limit exists. |

| Medical Consent Form is located on the second page of the Health History form, and needs-to be sent to (okay to fax) your doctor to be filled out and signed - BEFORE you can begin exercising. Of Course, it is always recommended that you have a physical prior to beginning any exercise program. |
