Parent Responsibilities
1. Please inform us if your child has been sick over the weekend, or has not slept well or was ill during the previous night, so that we can monitor your child more closely during the day.
2. Please honor the daycare Health Policies regarding illness in the daycare, exclusion from daycare; medication administration and authorization, and immunizations and history and physicals of your child.
Exclusion from Daycare
General Illness: If your child becomes sick during the day, they will be sent home. They must remain at home for the next day.
Rashes, Diarrhea or Vomiting: Unless otherwise noted by a physician in writing, all diarrhea or suspicious rashes are considered contagious. If you doctor determine, in writing, that the cause is not contagious, a child can return to the daycare. Otherwise, a child must be twenty four (24) hours without vomiting and diarrhea before returning to the daycare.
Fever: If your child has a fever of 101.0 degrees or above they cannot attend the daycare. Their temperature must be normal for 24 hours before returning to the daycare. Any temperature over 101.0 is not considered a "teething fever".
Cough: Coughs that are persistent, deep, croupy or congested, should not attend the daycare.
Discharge from the nose: A clear discharge may be from colds, allergies, or teething. If the discharge is moderate and can be attributed to teething or allergy your child will be allowed to attend daycare. However, if a child has a consistent runny nose, which is difficult from contaminating toys, clothing, etc. Your child will be sent home. If your child doesn't feel well and has a consistently runny nose, or nasal discharge that is cloudy and NOT clear, he should remain out of the daycare.
Special Situations: We will accept notes from the child's doctor for a child's return to daycare only after the doctor has physically examined the child. However, if your doctor's recommendations for return conflicts with our health policies, our policies will prevail over the doctor's recommendation for that particular situation.
These exclusion policies are for the protection of all children in the daycare. It is what you would expect of other parent to protect your child from unnecessary illness.
Other communicable diseases or conditions will be dealt with according to CDC guideline, for instance head lice, chicken pox, strep throat, etc.
Other Prescriptions and Non-Prescription Medication
All oral, topical, inhalant, and injectable medication in this category must have the following in place prior to administration: Completed written Parent/Guardian Permission & Prescriber Authorization on approved form. The authorization form will contain:
� name of child � date of birth � address � date medication order was written � medication name � dose and method of administration � time the medication is to be administered � dates the medication is to be started and ended � relevant side effects and authorized prescribers plan for management if they occur � notation if medication is a controlled substance � any allergies, reactions to, or negative reactions with food or drugs � specific instructions as to how the medication is to be given � name, address, and phone number of authorized perscriber � signature of authorized perscriber � name, address, phone number, signature, and relationship to the child of the parent giving permission for the administration of the drug by daycare staff.
Medication must be in the original child resistant safety container. The container or package must have the following information: Name of Child, Name of Medication, Directions for administration, and Date of Prescription (within one month for antibiotics).
Oral Medication: (For example)
Cold Medication (i.e., decongestant, cough syrup, etc.) Antibiotics (i.e., PenVK, Ceclor, etc.) Pain or Fever Medication (i.e.,Tylenol, Ibuprofen) Seizure Medication *i.e., Depakote, etc.)
Topical Medication: (For example) Diaper Ointments with antifungal (i.e., Mycolog, etc.)
Inhaled Medication: (For example) Asthma Medication (i.e., Proventil Inhaler, etc.)
Injectable Medications in a premeasured, commercially prepared syringe: Allergic Reaction Emergency Med (i.e., EpiPen Autoinject) Must only be for emergency treatment for a medically diagnosed condition
NO INVESTIGATIONAL DRUGS WILL BE ADMINISTERED IN THIS DAYCARE.
Parent Responsibilities
1. Submit a completed authorization form for the administration of medication. Authorization from prescribers is needed (as indicated above) and must be on an approved form and must be completed by the prescriber. All sections must be completed and, if necessary, "None", or "N/A" indicated where applicable. The form will not be accepted if there are blank sections and there is a question in the mind of the daycare provider.
2. The medication must be in the original container or package and must contain the information as described above.
3. Parent/guardians must administer the first dose of any medication. The daycare provider cannot administer the first dose of a medication except in an emergency.
4. Notify the daycare provider if there is any change in condition or if the order for administration of the medication has changed.
