Here is the information that you need to gather to create a family tree:

Start with a blank piece of paper, and add the following sections down, giving enough space for the following information to be added to it.

Your Name Your date of birth The place you were born Optional:Where you went to school, where you lived, etc.
Your Father His date of birth His place of birth His date of death (if applicable) Place of death & Cemetary (if applicable) Any optional items you would like		Your Mother (use maiden name) Her Date of birth Her Place of birth Her date of death (if applicable) Place of death & Cemetary (if applicable) Any optional items
His father His date of birth His place of birth His date of death (if applicable) His place of death & cemetary (if applicable) Any optional items	His Mother Her date of birth Her place of birth Her date of death (if applicable) Her place of death & cemetary (if applicable) Any optional items	Her Father His date of birth His place of birth His date of death (if applicable) His place of death & cemetary (if applicable) Any optional items	Her Mother Her date of birth Her place of birth Her date of death (if applicable) Her place of death & cemetary (if applicable) Any optional items

The trick to getting this form to work is to label the pages that you are using and when you need to link off of a page, you write the page to go to on the previous page.
