EMERGENCY EVACUATION BUS DRILLS
In accordance with New York State School Safety Legislation,
Raynor Country Day School is required to conduct at least
one emergency dismissal drill each calendar year. The drill
will calmly prepare the children for any type of early dismissal
that may occur as a result of an emergency situation.
In the event of an emergency, we would dismiss
the students as quickly and efficiently as possible. Emergency
Dismissal forms should be completed and kept on file for all
students.
MEDICAL EMERGENCY FORMS
Parents are required to complete a medical emergency form,
which lists all the information the school needs if a student
is taken ill while at school. Return the form before the first
day of school. It lists the following information: parent's
home and office telephone numbers; individuals the school
may contact if the parents are unavailable; and a standard
release authorizing emergency treatment should the need ever
arise.
Parents are responsible for immediately updating
the form whenever there is a change in information.
YEARLY MEDICAL EXAMINATIONS
Each student must have a yearly medical examination. Our health
form, which includes your child's immunization record, lead
screening and medical exam summary, must be completed and
signed by your doctor. The school nurse must receive this
form before your child may attend school.
IMMUNIZATION REQUIREMENTS
The New York State Public Health Law requires
that children entering school be adequately immunized. The
school nurse must receive proof of your child's vaccinations
before your child may attend school.
STUDENT ILLNESS
In order to ensure the health and safety of all children in
our program; please do not send a sick child to school. If
your child becomes ill during school hours, you will be notified
to pick your child up. First, we phone a parent or legal guardian.
In the event we are unable to contact you, we follow the list
of emergency numbers provided. We urge you to keep your list
of emergency contacts up to date, leaving at least one number
where you can be reached during school hours, and
providing names and phone numbers of at least two additional
individuals authorized to pick up your child in the event
you cannot be reached. Your child will remain in the nurse's
office until someone arrives. We require that the child be
picked up within one hour of notification. The sick child
must be signed out at the front desk before he/she will be
released. Upon returning to school, a note explaining the
absence will be required.
A CHILD WITH A MEDICAL OR PHYSICAL CONDITION
Please advise the school if your child has a physical or medical
condition that may require attention during school hours.
Raynor Country Day School requires doctor's instructions.
Medical equipment or medication prescribed must be made available
to the school should the need arise to use it. If the condition
affects the child's participation in any school activities,
please inform the school nurse.
ADMINISTRATION OF MEDICATION
All parents must advise Raynor Country Day School of any medication
given to a child prior to coming to school, prescription or
non-prescription. All medication to be given during school
hours must be authorized in a written statement signed by
the physician and parent. This includes, but is not limited
to, over-the-counter medication such as Tylenol and Advil.
This statement must include:
· name of medication
· dosage
· reason for administering
· times to be administered
· possible adverse reactions
Medication consent forms to be completed by
your physician are available at the front desk.
All medicines must be in their original containers
and should be brought directly to the school nurse. If your
child rides the school bus, please hand medications directly
to the driver.
Please do not store medicines in your child's
backpack or cubby.
