Health Guarantee Contract

The seller, Shelley Hudak, warrants that this Sheltie puppy is in good
health and free from disease (including Parvo) and/or genetic defect. We
recommend that buyer take puppy to a veterinarian for an examination
within 72 hours from the date of pick up. The health of the puppy is only
guaranteed for 72 hours after leaving my premises.

Buyer is responsible for continued vaccinations. FAILURE TO CONTINUE WITH
VACCINATION SCHEDULE AND OR ROUTINE MEDICAL CARE WILL VOID THIS AGREEMENT.

Genetics:

Shelley Hudak will replace the puppy if it has been determined to have a
genetic defect that would cause death of the puppy. No replacement will be
given without documentation by a licensed veterinarian and documentation
is the responsibility of the buyer.
Shelley Hudak is not responsible for any illnesses that the puppy could
acquire after leaving our premises. Instructions have been given on the
feeding schedule of the puppy and its daily routine.

This agreement is voided if any type of abuse and/or neglect is evident or
can be shown to exist or have existed. This agreement does not cover
accidental injury or death.

Shelley Hudak is not responsible for puppy if it is not checked by a
veterinarian within 72 hours. It is very important for the puppy to be
checked by a veterinarian with in 72 hours.

A non refundable deposit of $250 is required to reserve a puppy. If full
payment is made at time the puppy is ready to leave and buyer backs out of
sale the payment will be refunded minus $250.

WE DO NOT SHIP OUR PUPPIES!

A NON REFUNDABLE DEPOSIT OF $250 IS REQUIRED TO HOLD PUPPIES.
