If you are having trouble paying for your prescriptions then we can help you. We can get some of your medicine free and some of your medicines will cost less. Some prescriptions are not covered by any of the programs available. We will do our best to help you. This is the procedure to get help.

The first thing you need to do is to meet with a volunteer of PALS and give that volunteer the data they will need to fill out. Click HERE to get a copy of the patient form or go to Dr. Birdsall's office and pick up a copy of the patient form. Read it carefully and please fill in as much as possible. The company will not accept the applications if the information is not correct. Please print all of the entries. Make sure the medicine list is correct. You do not have to fill in the company who makes the medicine. We will look that up after we go over the patient form.

Next, you will need to get a proof of Income statement. If you filed income tax last year then you must bring in a copy of your forms. If you did not file Income tax then bring a copy of your check. Or, the paper you received at the end of the previous year(2003). If you don't have the previous forms and your check is direct deposited then ask the bank teller to give you a copy of the direct deposit." They will know what to give you. Call Dr. Birdsall's office (632-6233) and make an appointment to speak to a volunteer of P.A.L.S. Appointments are usually scheduled on Tuesdays but if this is a problems, someone will come in the get your information.

Bring the complete patient form and the proof of income to the volunteers of P.A.L.S. The volunteer will find the company that makes your medicines and she/he will find the application forms of the program that will give you the medicine free or at a reduced cost. You will have to sign all of the application forms before you leave.

The volunteer will fill in all of the information on the applications using the patient form you filled out. When this is complete, the volunteer will take all of the filled applications to the doctor and they will write the prescriptions and sign the form.

After the doctor signs the forms and writes your prescriptions, the volunteer will check it to make sure all information is correct then it will be mailed off to the company. If you are approved or not, the company will send you and the doctor a letter to inform you. If you are approved, you will get your medicine in 4 to 6 weeks.

When your medicine comes in, a volunteer will check it to make sure it is correct. You will be called to tell you your medicine is in. You will receive a 3 month supply of your medicine. Mark on your ca lander two months from that day and you will have the date that you should call the office for a refill. [Example: If you get the medicine on 10-10-2004, you would go to the month of December and mark the name of the medicine on 12-10-2004, 2 months later, to remind you to call in for your refill.]You can pick up your medicine anytime the office is open. Most people get all of their medicines free or at least cheaper.

If you still have questions about the program, called the doctors office (632-6233) and ask to speak to P.A.L.S. and if one of the volunteers is in, they will help you. It seems complicated but its not. Your pals at P.A.L.S. will help you.
