As part of my Team CIA class, we made a survey. It would be great if you can take five minutes of your busy day to fill it out and send it in.

To create this survey, i inserted the form elements the same as with the practice survey. In a table with two columns, on the left is the question, and on the right is the form element for the answer. To name the form element, right click on the form element. Then, click on form field properties. Insert the name that you want (without spaces) in the name area. Do this for all the form elements. Right click on the submit button. Then, click on form properties. Click on options. In the action cell, type in /cgi-bin/homestead/mail.pl?kicks327.
