Ahhh... yes, for the first time we get to see the more violent side of Carl. I had a hard time trying to get the scene in panel 4 to look normal. The first time I drew it Carl's and the Intern's arms look shocking disproportionate! I still think it looks a bit funny. But that's the best I can do lah. 

Although unlikely in real life but what is true though is the alarming frequency at which I do my "clerking" using the method as described by the intern above. Of course I would justify myself by trying to see the patient and talk to him/her in person as well. I think this way it really helps me to understand the case much better although it is technically considered cheating since you won't be able to read the notes under exam conditions.
