It is obvious who is the "patient" and who is the "doctor". The doctor, covered in spots, wields a Ngulu held over his head. The patient would either be seated in a makeshift stool that is staked to the ground as in the example above, or have their hands tied behind their back and staked to the ground. A sappling would be bent down and tied to the patient's neck to elongate the neck and facilitate a cleaner cut.
