Myxedema in patient of hypothyroidism

 

The clinical features of hypothyroidism are variable, many a times nonspecific. 

A high index of suspicion is necessary especially in patients who are at risk for developing hypothyroidism but still are subtle in their presentation. 

The clinical features vary with age of onset and severity of hypothyroidism

 

Clinical manifestations of hypothyroidism in general / in adults.

Fatigue

Lethargy 

Mental impairment 

Depression

Goiter

Non-pitting Edema (swelling of body, limbs which does not pit on pressure) 

Menstrual abnormalities in women

Cold intolerance

Hoarseness of voice

Repeated abortions in productive age of women.

Dry skin

Weight gain 

Arthralgias (pain joints)

 

Hypothyroidism during adolescence

Delayed puberty

Growth retardation with delayed bone age

Galactorrhoea(secretion of milk at nipple)

Menstrual disorders in adolescent girls

 

Hypothyroidism during childhood

Dry skin, generalised myxedema

Constipation

Delayed dentition(erruption of teeth)

Growth retardation(faluire to grow)

Delayed skeletal maturation(bone growth)

Myopathy(muscle weakness)

Precocious sexual development(abnormal sexual development)

 

Clinical manifestations of congenital hypothyroidism

Early neonatal hypothyroidism

Prolonged icterus (jaundice at birth)

Edema (swelling of body )

Birth weight > 4 kg

Post datism : gestation > 42 weeks

Poor feeding, hypothermia(decreased body temperature)

Abdominal distension

Large posterior fontanelle(soft area in skull of new born)

 

Onset during lst month

Peripheral cyanosis / mottling(blueness of body)

Respiratory distress(breathing difficuilty)

Failure to gain weight

Constipation

Decreased activity

 

Onset during first 3 months

Umbilical hernia (protrusion of abdomen at navel)

Constipation

Hoarse cry

Macroglossia (large tounge)

Generalized myxedema (swelling of body)

Dry skin
