5.0 Result of Incident Investigation
5.1
Initially a general inspection was made of the escalator to ascertain
if any obvious fault conditions
existed relating the incident. The comb plates were checked and
showed the following:
Upper
Landing - All of the combs and comb teeth were in tact, complete
and securely fastened Lower Landing - All of the combs were securely
fastened. Numbering the combs left to right facing upwards,
and commencing with the first comb on the left

The upper and lower comb teeth meshed into the step grooves by
approximately 4 mm

The distances between the steps and skirt panels varied between
a maximum
of 8,5 mm and a minimum of 2 mm

Both the handrail speeds were measured and found to be correct
5.2
Initially half of the escalator steps were removed for further inspection.
During this period a step
chain roller was found on the lower part of the enclosure pan where
the incline meets with the horizontal
5.3
The inference may be made that
the step chain roller could have dislodged as a result of
a retainer clip failing. Even though the remainder of the escalator
steps were removed it was
impossible to find the broken circlip to test the integrity of the
material.
6.0 Corrective Actions
6.1
All the retainer clips pertaining to the step chain rollers where
examined for any noticeable
damages and as a preventative measure, replaced irrespective of
wear.
6.2
The escalator was examined according to SABS1543.1991 and results
verified in Annexes A, B,C.
Record copies were distributed to the Department of Labour, Sandton
Sun Hotel, and to the site
6.3
One escalator step was found with damaged step treads. This step
is presumed to have been
involved in the incident and the entire step was replaced
