Clinical Study Day and Annual General Meeting 2000

Welcome to all. The following information is meant to guide you through the activities related to the Annual General Meeting (AGM) 2000 and the DMA Clinical Study Day including the Pre-AGM Council meeting, the Meet and Greet and the Mess Dinner.
HOTEL
A number of rooms have been reserved at the Hampton Inn, 100 Coventry Road, Ottawa, Phone 1-800 HAMPTON (426-7886) or locally 741 2300, e-mail [email protected] . It is located at the corner of Coventry Road and the Vanier Parkway and is just across the street from the RCMP HQ and next door to Ottawa's JetForm Baseball Park. It is the hotel facility closest to NDMC.
AGM & CLINICAL DAY.
The AGM and the Clinical Day will be held in the Auditorium, National Defence Medical Centre, 1745 Alta Vista Drive. The AGM will take place on Friday, 20 Oct 2000 and the Clinical Study Day on Saturday, 21 Oct 2000. The Reception desk will be opened as of 0800hrs. Both activities will start at 0900hrs and will be over early afternoon. The Clinical Day is accredited for Continuing Medical Education and a certificate of attendance will be issued on payment of the registration fee.
REGISTRATION FEE
The AGM of course is free of charge. The Clinical Day is $80.00, which includes the coffee breaks and a light lunch. You may also register for the Mess Dinner which will take place Friday evening at a cost of $55.00 for a total registration fee of $135.00.
REGISTRATION FORM
The registration form is attached. It you plan to attend please complete the form, enclose your cheque made to the order of The Reception Desk will be located in the foyer, next to the Auditorium. It will be manned all day from 0800 hrs 20-21 Oct 00.
Those who plan to attend the AGM may purchase coffee and a light lunch in the Snack Bar available on the ground floor of NDMC. For those attending the Clinical Day, coffee breaks will be served in the main foyer and a light lunch will be provided in the Social Centre located on the First Floor.
On Friday, you may park in the area marked VISITORS on the South side of the building but you must register your car with the Commissionnaire at the main entrance to the building and pay a $3.00 parking fee for a day pass which must be displayed in your windshield. On Saturday all of the parking lot is available free of charge.
The Meet & Greet will be held at the Hampton Inn, in the suite reserved for the DMA of Canada, on Thursday 19 Oct 00 following the Pre-AGM Council Meeting, which should be over by 2000 hrs. Of course all are welcome to drop in and have some refreshments at any time on Thursday. We will endeavour to have the suite available, as a meeting and gathering place, as much as possible throughout.
The Mess Dinner will be held at 1900 hrs for 1930 hrs on 20 Oct 00 in the Ottawa Warrant Officers' and Sergeants' Mess located at # 4 Queen ElizabethDrive. Cost is $55.00 per person. Parking is available near by at the Provincial Court indoor parking lot (entrances off Laurier Street and Elgin Street) or park on the street if you can find a place. Please register early and, no later then 01 Oct 00 as there are only 120 places. You may check with the Reception desk on arrival, for late registration, which will be on a first-come first-served basis providing there are still seats available. Dress for the Dinner is Mess Kit, Service Dress, tuxedo or business suit with bow tie. Cocktail dress for ladies.
This meeting, of all Branch Presidents along with the National Executive Committee, will take place in the Suite reserved for DMA at the Hampton Inn on Thursday, 19 Oct 00 at 1730 hrs.
Sorry NO REFUNDS can be made for cancellations, for either the Mess Dinner or the Clinical Day, received after the 13 Oct 00 since funds will already have been committed. A small administrative fee may be charged for refunds of registration fees if notice of cancellation is received prior to13 Oct 00.

Il se trouve au coin de la rue Coventry et de la promenade Vanier. L'h�tel fait face au QG de la Gendarmerie Royale et tout pr�s du terrain de baseball JetForm. Cet h�tel est le plus proche du Centre M�dicale de la D�fense Nationale (CMDN).
L' AGA et la Journ�e Clinique auront lieu dans la salle de conf�rence du CMDN, 1745 Alta Vista. L�AGA aura lieu, vendredi, le 20 octobre 2000 et la Journ�e Clinique, samedi, le 21 octobre 2000 et les deux commenceront � 09h00 pour se terminer en apr�s-midi. Les pr�pos�s � l�accueil seront en fonction � compter de 08h00. Les participants � la Journ�e Clinique recevront une attestation de participation qui leur permettront de se pr�valoir des cr�dits pour la formation professionnelle continue.
Il n�y aura aucun frais pour participer � l�AGA. Les frais d�inscription pour la Journ�e Clinique seront de 80 dollars ce qui comprend les pauses caf�s et le repas du midi. Vous pouvez vous inscrire pour le D�ner r�gimentaire, qui aura lieu vendredi soir, pour 55 dollars suppl�mentaires par personne.
L formulaire d�inscription est joint. Veuillez le remplir, y joindre un ch�que fait � l�ordre de � l�arriv�e veuillez vous pr�senter � l�accueil, situ� dans le foyer, pr�s de la porte principale menant � la salle de conf�rence. Vous y trouverez des pr�pos�s � compter de 08h00 les 20 et 21 octobre 2000.
Les membres qui participeront � l�AGA pourront acheter un caf� ou un repas l�ger au casse-cro?te situ� au sous-sol du CMDN. Pour ce qui est de la Journ�e Clinique on prendra les pauses-caf� dans le foyer principal et le repas du midi sera servi dans le Centre Social situ� au premier plancher.
Pour assister � l'AGA il faudra utiliser le stationnement des VISITEURS situ� au c�t� sud de la b�tisse mais il faudra, en plus, vous rapporter au Commissionnaire, � l'entr�e principale afin d'obtenir un permis au co?t de 3 dollars pour la journ�e lequel devra �tre fix� au pare-brise de votre voiture. Pour la Journ�e clinique le stationnement est gratuit et vous pourrez vous stationner o Cette activit� aura lieu � l'Hampton Inn dans la suite r�serv�e pour l'AMD du Canada, jeudi soir apr�s la r�union du Conseil avant l'AGA. Cette r�union devrait se terminer vers 20h. Bienvenue � tous et nous esp�rons que vous viendrez en grand nombre renouer de vieilles amiti�s et faire de nouvelles rencontres. La suite sera ouverte apr�s les activit�s de la journ�e du 19 - 21 octobre alors venez-y faire un tour et prendre un verre ou tout simplement jaser.
Ce d�ner aura lieu le 20 octobre 2000, � 19h00 pour 19h30, au mess des Adjudants et des Sergents d'Ottawa lequel est situ� au 4, promenade Elizabeth. Il en co?tera 55 dollars par personne pour y assister. Vous pourrez vous stationner dans le parking de la cour provinciale � lequel on peut acc�der par la rue Elgin ou la rue Laurier. Vous pourrez aussi vous stationner sur la rue si vous �tes assez chanceux pour vous trouver une espace. On vous conseille de vous inscrire t�t pour le d�ner et de toute fa�on au plus tard le 1 octobre 2000 car il est limit� � 120 personnes. Vous pourrez toutefois v�rifier aupr�s des pr�pos�s � l�accueil lors de l�arriv�e, pour une inscription tardive, mais ce sera le premier arriv� le premier servi et ce s�il reste de la place. La tenue est�: militaire de gala, militaire de service, smoking ou complet et noeud papillon. Pour les dames robe coktail.
Cette r�union, des pr�sidents de toutes les sections de l'AMD et du Comit� Ex�cutif national, aura lieu, jeudi le 19 octobre 2000 � 17h30, dans la suite de l'AMD du Canada � l'h�tel Hampton Inn.
Nous regrettons de ne pouvoir rembourser les frais d�inscription pour la Journ�e Clinique ou pour le D�ner r�gimentaire � moins que l�annulation n�ait �t� re�ue avant le 13 octobre 2000. � noter qu�il pourrait y avoir des frais d�administration pour tout remboursement de frais d�inscription.
