Here are some samples of procedures that might be considered for inclusion in a Behavior Support Plan.  Please understand them to be samples drawn from other plans I have written and in need of some editing to be used for another person.  The samples overlap.  Several have multiple versions of the same data form.  The intent is to provide options to consider. At the very end of the list you will find a file including all the samples in one big file.  The files are all Word documents that will first need to be unzipped.  They are in merge format (i.e., firstname lastname and heshe), but need to be activated.  The data document is available.  Download, unzip, and save the data doc first before activating  the merge doc.  Feel free to remove my headers and footers and edit for use as you see fit.  Use with permission.  All I ask is for a little credit and some feedback via e-mail.

1.
Requests For Respect & Support -- guidelines for interacting with firstname lastname written from hisher perspective, i.e., our hope and best guess about how heshe would say heshe would like to be treated if heshe could say.  These "Requests" were the forerunner for the Personal Support Plan

2.
Gentle Teaching -- application of approach to increase participation in activities and teach value of doing things together.

3.
Communication and Choice Making Using Objects -- starter plan for teaching communication using the most basic and concrete language, i.e., exchange of objects.

3A.

3B.

5.
Start-Up Picture Communication Training -- system for starting communication training using general; "I Want ..." and "Leave Me Alone" Cards.

6.
Participation Facilitation Using Pictures -- system for encouraging participation in activities, choice making, communicating and interacting using a picture communication system.

7.
Social Skills Training Sessions -- example of more formal procedure for beginning communication training using objects in a controlled situation.

8.
Facilitated Expression -- includes several methods for encouraging expression of thoughts, needs, and feelings.

9.
Discussion Notes -- to be used on a routine basis to provide practice and reward and at other times to resolve problems and prevent escalation
