Wow... two episodes in a row that include major plot points from the manga. I wonder how long they'll keep this up for? (Note: Just one more episode.)

A villain sends an attack towards Sailor Moon. Tuxedo Mask pushes himself in front of her and takes the blast himself. He is on the verge of death. She cries, and transforms into Princess Serenity as her tear forms into the Silver Crystal. In the manga, it happens in Act 8 and Act 9. In the anime, it happens in Episode 34. (There's a slight difference in the anime, in that the seven Rainbow Crystals fly to where Usagi is and merge with her tear to form the Silver Crystal.)

Here's where things diverge: In the anime and manga, Mamoru is then captured by the Dark Kingdom (and subsequently turned evil). However, in PGSM she heals Mamoru using the Silver Crystal, which emits a bright glow. As well as Usagi realising that she is the Princess, Mamoru realises that she is the Princess from his dreams.

Sailor Venus's Princess tiara being replaced with an ordinary Sailor Guardian tiara is similar to in the manga, where the crescent moon mark on her forehead was replaced with the ordinary tiara.
