NEW STUDENTS

1.)  A new student must present himself/herself to the guidance office for interview.

2.)  He/She must submit the following document to the registrar’s office:

  • Original copy of Form 138 (High School Card);
  • Original copy of certification of good moral character;
  • Certification of the honors obtained, if valedictorian or salutatorian, and the number of graduates, signed by the Principal of the high school where he/she graduated;
  • Three (3) copies of 2" x 2" photo;
  • Xerox copy of Birth Certificate.

3.)    The person in charge in the registrar will give you a registration form, fill it up with the schedule subjects prescribed for the semester.

4.)    Present the accomplish registration form to the Department head for approval.

5.)    Proceed to the registrar's office for signing and for final checking of subject requirements.

6.)   Once the registration form approved and checked, proceed to the accounting office for assessment.  Then payment of tuition fees will be at the cashiers office.

7.)    Present the payment receipt to the registrar's office for securing of registration card.  Fill it up with the schedule of subjects prescribed for the semester.

8.)   Secure the signature and approval of the Department head and guidance office on the registration card for male students, secure the approval of the ROTC office.

9.) Submit the accomplish registration card to the registrar's office for signing.

11.) Present the registration card to the accounting office for official enrolment.

12.) Proceed to the registrar's office for the issuance of classcards.
