A. New Students

• Original copy of Form 138 (High School Card);
• Original copy of certification of good moral character;
• Certification of the honors obtained, if valedictorian or salutatorian, and the number of graduates, signed by the Principal of the high school where he/she graduated;
• Three (3) copies of 2" x 2" photo;
• Xerox copy of Birth Certificate.

B. Transferees

• Honorable dismissal from the school previously attended;
• Transcript of scholastic records;
• Certificate of good moral character
• Should pass an interview conducted by the office of the President.
• Three (3) copies of 2" x 2" photo;
• Xerox copy of Birth Certificate.

C. High School

• Original copy of Form 138;
• Original copy of certification of good moral character;
• Four (4) copies of 2" x 2" photo
