This is a form created for GTT Express Transportation, as a new towing company they needed to have a way to track vehicles condition before and after transit. The finished pieces are printed on two part carbon-less paper.

This project was apart of a class assignment, we had the opportunity to work with Cystic Fibrosis Foundation as a client. As a class we created several pieces for their events, this piece started as a group project with my oversight, over the course of the term I took over and finished the piece. The final version is a self mailing tri-fold invitation.
