Where to buy?

This memory module can be purchased directly from Crucial Technologies.  It is part number is CT4M64S4D7E.  The chips can be desoldered from this module.  The module also has surface mount capacitors on it.  There are sixteen 0.10uF +-20%* brown capacitors on this module.  There are also some other component which look like the capacitors which aren't capacitors.  These are either gray or off-white.  The module contains one 47K resistor which may possibly be used for R84.  Use of this resistor for R84 has not been tested yet.

What's the catch?

You have to desolder the chips to remove them from the module.  I can't really speak for Crucial, but I'm pretty sure that removing the chips from the module voids the warranty ;).  Memory chips can easily be destroyed by improper handling or installation.
