Payments must be made in cash only.
Steve Semak is a Primary Care Physician. This means you can submit a claim to your medical insurance for reimbursement. Your insurance may cover all or part of your treatments. If asked, we will provide for you a print out with the information needed for your claim. Although we will use standard insurance codes and customary fees on your print out, how much you are reimbursed is between you and your insurance company.
We accept some California Workers Compensation, Personal Injury, and Private Insurance cases.
Whether you receive acupuncture, herbal medicine consultation, weight control consultation, massage therapy, physical therapy, therapeutic exercise, cupping, moxibustion, counseling, lectures, workshops, ear coning, microcurrent rejuvenation or any other modality or combination of these modalities provided by Steve or his staff, fees are based on the time of treatment.
The fees are:
$100/ 60 minutes
$140/90 minutes
$175/120 minutes.

Sign up for a Prepaid Package Plan and receive a 25% discount for 10 Treatments and a 30% discount for 20 Treatments.
Treatments can be done at your home or another location of your choosing.
An additional Excessive Mileage Fee will be required for travel over 45 miles from Simi Valley. The Excessive Mileage Fee may be waived if we are able to schedule with another patient in your area for the appointment prior to or after yours. Also, if you are in a hotel or other location and there is a parking fee, that will need to be added onto the total cost of treatment.
Arrangements can be made for hardship cases.
