If you pay by personal check*, cashier's check or money order, please fill the form out, print it and mail it to: Gwyn Sutherland, 891 White Mills Glendale RD W, Glendale, KY 42740.

If you pay with PayPal, fill out the form and e-mail a copy to me, then click on PayPal to complete the transaction. My PayPal ID is [email protected]. If you do not have a PayPal account, you can sign up for one by clicking the ad bar at the bottom of the page.

*Order will be held for 10 days in order for check to clear. Returned checks will be submitted to appropriate law enforcement personnel.

Total the amount of the order, add $6.95 for one item (add $3 for each additional item) and submit that amount. Orders with incorrect payment will not be filled. E-mail me first, if you have any questions. Orders will be shipped via USPS. Insurance is $1.60 extra up to $50 value. Current USPS rates will apply for amounts over $50. Out of country orders will cost more to ship and must be paid in US funds.
