FORMS AND FORMAT TO BE UTILIZED BY THE CONTRACTOR

1. APPLICATION & CERTIFICATION FOR PAYMENT - HUD Form 51000 - 51004 are used to submit pay a pay application once the "pencil copy" is approved.

2. APPLICATION & CERTIFICATION FOR PAYMENT (PENCIL COPY) - AIA Document G702 and G703 are used as a "pencil copy" and should be included with the actual pay application once approved.

3. CONSTRUCTION PROGRESS SCHEDULE - HUD 5372, or acceptable substitution, is to be submitted to the Architect/Engineer, in triplicate, for approval.

4. CERTIFICATE FROM CONTRACTOR APPOINTING OFFICER OR EMPLOYEE TO SUPERVISE PAYMENT OF EMPLOYEES - HUD 5282 is submitted to SMHA Construction Department within 10 days of contract signing.

5. REQUEST FOR ACCEPTANCE OF SUBCONTRACTOR - HUD 5371, and NON-COLLUSIVE AFFIDAVIT - MHA 116, are submitted to SMHA for approval together with a current Certificate of Insurance (naming SMHA as Additional Insured) three days PRIOR to commencement of work.

6. PAYROLL FORM - WH 347, or computer generated equivalent, is submitted on a weekly basis by the general contractor and subcontractors as well. All payrolls for the pay application requested must be submitted prior to payment.

7. RELEASE OF LIEN (partial/final) - MHA 108 must be submitted with each application (beginning with number two) from each subcontractor/supplier.

8. SUBCONTRACTOR ADMINISTRATION MEMO - MHA 230, for your reading pleasure and information.

9. NOTICE TO ALL EMPLOYEES - WH Publication 1321, must be posted in a conspicuous location so as to be visible to all employees working on the project.

FOR SUBMISSION OF

CLOSEOUT DOCUMENTS

The following is required to document the close out of this project:

L Maintenance Guarantee (MHA 118), Submitted in duplicate with original signatures.

L Affidavit for Final Payment (MHA 119), Submitted in duplicate with original signatures

L Contractors Certificate and Release (MHA 120), Submitted in duplicate with original signatures

L Letter of Completion. This letter must appear on your company’s letterhead and state the project and date completed.

L Certificate of Occupancy. Submissions required for Residential Rehabilitation only (5h projects).

L Lien Waiver and Release (MHA 108), Required for each subcontractor/supplier who has supplied labor/materials on this project. This Lien Waiver MUST be completed on the MHA form provided and indicate a final payment has been received.

L As Builts (As required)

L Warranties (As required)

L Final Pay Application (HUD 51001 - 4) for retainage amount only.
