Program Objectives

 

Getting Things Done

1. Members will inform 1,250 members of the low-income, underserved, primarily Latino community of available dental and medical health care services and assist at least 80% of the stated population to enroll in an health insurance plan (BHP, CHIP, Healthy Options) and become oriented to clinic services, resulting in their obtaining a medical home. Measured by the numbers of community members assisted and enrollment data.

2. Members will promote health awareness for 1,000 individuals through education programs, assistance with self-care, and my encouraging proper health maintenance schedules with the primary care provider. 65% of these individuals will show increased knowledge through pre/post client evaluations.

3. Members will provide books and volunteer readers for 2,000 children in the Seattle, Mt. Vernon, Smokey Point, and Tacoma clinics, with 85% of those children showing increased interest in reading, and/or 85% of the parents showing an increased understanding of the developmental importance of early childhood literacy, as measured by distribution statistics, post-reading session feedback form, and provider observation.

Member Development

  1. Members will gain training on the National Service network and on Core Competency topics including health centers, outreach, health education, and conflict mediation. Cultural diversity will be emphasized to enhance sensitivity skills and to develop cross-cultural capital, as measured by in-service and other training documentation.
  2. Provide members with increased knowledge and skills to carry out service commitments through site supervision and support. 90% will receive at least satisfactory performance ratings by their site supervisors, and 80% of members will report satisfaction with training and increased competence at an exit interview with the Program Coordinator.
  3. Provide members with exposure to health care settings and an exchange of skills and knowledge among community health workers, health professionals, and students of the profession so that 75% will pursue higher education and/or careers in health and health related fields.

Strengthening Communities

  1. Members will develop and strengthen partnerships with other healthcare and minority organizations by participation in community events, resulting in collaborative service agreements with six agencies, as documented in meeting minutes and agency participation statistics.
  2. The program will utilize AmeriCorps members to train 25 community volunteers to provide translation and other services for 800 non-English speaking patients and their families, as measured by volunteer logs and enrollment records.
