Survey Instructions:
  1. Make a copy of this template for each student .
  2. Have students enter the number of servings eaten from each food group each day from Monday through Friday.
  3. Have students calculate their average servings for the week for each food group.
  4. Calculate the class average (average the student averages) for the week.
  5. Go to the Survey Data Form and enter the averaged data for your class.
