







Sandy's House FCC 
Welcome to my child care home! Sandy's House has been licensed by the State of Illinois for the care of children from 6 weeks to 12 years. This means I must maintain the standards for a licensed day care home regarding safety, nutrition and education. My program is Accredited by the National Association For Family Child Care. My home is equipped with smoke detectors, fire extinguishers and electrical safety plugs. A first aid kit is available. Safe play areas indoors and out are provided for your child.
Family child care is my choice of profession. I am not a "babysitter." I provide an environment that is comfortable, accepting and nurturing. I guide and teach. I am CPR and First Aid certified. I participate in the Federal Child Care Food Program. I belong to professional child care organizations at the local, state and national and international levels. I regularly attend conferences,training sessions, classes and seminars to upgrade my skills and provide the best quality care for your child.
PAYMENTS AND ABSENCES
Sandy's House is open Monday through Friday. Times are individually contracted according to the parents� work schedules.
A $200 fee will be charged and held as a deposit before care is begun. It will be applied to your weekly fees upon proper termination of the contract.
A four week trial period will be used to evaluate the childcare arrangement. Care may be terminated by either parent or provider at any time during this trial period. At any time thereafter, child care may be terminated by either parent or provider by giving a minimum of 30 days notice. Payment is required for the full 30 days even if services are not used.
Weekly child care fees are paid by electronic funds transfer from the parents' checking or savings account. In the event of a NSF return of an electronic transfer, a $5.00 per day late fee will be charged and full payment will be expected in cash. Additionally, the parent will be held responsible for any bank charges incurred by the provider. Should a debit mistakenly occur due to provider error, the provider will be responsible for any bank charges incurred by the parent.
Overtime charges will apply both before and after your individually contracted hours. Fees are $1.00 per minute and are payable in cash no later than the following Monday.
A $30 summer activity fee for children ages one and over is due by May 15th of each year. This may be paid by check or cash.
The provider reserves the right to refuse care if child care fees, late fees, NSF fees or any other fees are not paid as agreed upon, until such payments are made in full.
The State of Illinois requires that authorities must be contacted should a child be left at a childcare facility after business hours. The provider will attempt to reach a parent by phone and if unsuccessful, call the people listed on the child pick-up list. If no contact and arrangements have been made by 6:00pm, the police and DCFS will be notified and the child will be turned over to these authorities.
PAPERWORK
The State of Illinois requires that all forms are completed, signed and returned prior to the first day of child care. Paperwork must be updated as needed.
If at any time you cannot be reached at the work phone number I have on file, I must be informed and supplied with a number at which a parent can be reached.
No child will be released to anyone other than those listed on the Child Pickup form.
VACATIONS AND EMERGENCIES
Sandy�s House will be closed for three weeks of unpaid vacation time per contract year. Dates will be announced well in advance.
Should an emergency occur that forces the closure of Sandy�s House for more than 2 days, tuition will be prorated.
HOLIDAY, CONFERENCE AND PERSONAL DAYS
Sandy�s House closes for several paid holidays each year, as well as five paid professional training days and three paid personal days.
Holiday closings include:
New Years Day
If a holiday falls on a Saturday, Sandy�s House will be closed for observance on the preceding Friday. If a holiday falls on a Sunday, the child care will be closed on Monday.
SUBSTITUTE CARE
Substitute care will occasionally be used in Sandy�s absence. Every effort will be made to notify parents in advance. All substitutes meet the criteria set forth by State regulations.
CLOTHING AND SUPPLIES
A complete change of clothing must be provided for each child. These may be brought daily or left at Sandy's House. If your child is being toilet trained, extra changes of clothes and training pants will be needed.
All diapers, lotion, ointments and bottles are to be provided by the parent. Sandy�s House will provide baby wipes.
Closed toe shoes are preferred. If your child arrives in sandals, please also provide shoes and socks for the day, in case they are needed.
Suitable outdoor gear must be provided each day. This may include coat, boots, mittens, hats.
DAILY SCHEDULE
Arrival and pickup times are individually contracted and must be adhered to unless prior arrangements have been made. Sandy�s House accepts children for full days only. Please schedule doctor and other appointments for your child near the end of the day, or plan to keep your child home that day.
Care is provided while parents must work. Quality childcare is not a substitue for parental responsibility. With few exceptions, it is expected that your child will not be in attendance at Sandy's House when a parent is not working.
Meals are provided at no additional charge and are served at regular times, as mandated by the Federal Food Program.
Children must be in attendance at these times to receive the meals.
BREAKFAST 8:00am
Infants will be fed on demand.
Daily activities will include free play time both inside and out, art, science, math, field trips, reading, music, games, puzzles, etc, all within a flexible time schedule.
All children at Sandy's House will nap. Children who no longer need to sleep (generally five years and up) have a quiet time. Infants will nap as needed.
BIRTHDAYS
ILLNESS
Examples of conditions that would preclude a child from attending include, and are not limited to the following:
fever over 100 diarrhea vomiting infections earaches
contagious or undiagnosed rashes contagious diseases
extreme (other than clear) nasal discharge excessive coughing
Children must be free of symptoms for a minimum of 24 hours before attendance at Sandy's House.
Sandy's House will admit children with mild cold symptoms providing no over-the-counter fever medicines (such as Tylenol or Motrin) have been given. These medications can mask fever or other complications that would require exclusion from childcare.
Certain contagious diseases will require a doctor�s note before the child may return to daycare.
If a parent is home with an ill child it is expected that the enrolled sibling be kept at home also. Please keep the germs at your house! Young children touch and mouth everything and can easily pass germs from their homes to their playmates at childcare. We have unfortunately, had many incidents of illness being passed to the other children via their "healthy" siblings.
In addition, a child who does not feel well enough to fully participate in all of the activities of the day is not well enough to attend childcare.
In the event your child becomes ill while at Sandy's House, you will be notified and will be expected to pick up your child within one hour. In the meantime, your child will be kept comfortable in a room away from other children.
Please call as soon as possible if your child will not be in attendance due to illness.

Please respect my neighbors. Do not block driveways or let your children trespass or cause damage.
For safety reasons parents must maintain control of their child when arrivi
ng and leaving Sandy�s House. Please take your child by the hand, if necessary.
Under no circumstances should children be unattended in a car.
At no time will children:
run, jump, wrestle, climb, etc. in the house or on the furniture.
hit, kick, bite, push or otherwise hurt anyone.
throw or intentionally break anything.
pick up babies or toddlers

Children should arrive at Sandy�s House dressed in play clothes. We do get dirty!

We have a small celebration for birthdays. Please bring a treat to share on your child�s special day. Treat bags, birthday plates and napkins, etc are optional. Other families may choose to bring a small gift for the birthday child.
Sandy�s House provides well child care and sick children cannot be accommodated.
Parents will drop off and pick up their children in the front entryway.
I have received a copy of the policies and rates and agree to follow these rules and pay according to the rate schedule.
I understand that failure on the part of the provider to enforce any part of these policies does not constitute a change in policies nor does it negate any portion of the contract.
I understand that the contract and policies may be ammended by the provider with a minimum of two weeks notice.
I understand that the proivider is a mandated reporter of suspected child abuse or neglect.
Termination voids this contract.
