You have a Custom Avatar Form....
Now What?
If you do not have a Custom Avatar Form, you must first purchase one.
Go to the Mr. Store in town and trade five Mr. Accessories to get one.
If you already have a Custom Avatar Form, continue reading.
1) You can submit the form with a description of what you want. You must follow the Custom Avatar Guidelines. Anything that requires original artwork or animation is almost guaranteed to be rejected.
2) Learn to draw avatars on your own.* The popular programs are Adobe Photoshop, Adobe Image Ready, Flash and Paint.
3) Hold a contest in the KoL Contests forum.** Your post should include the following information:
     a) a description of what you want the image to look like

b) the prize(s) you will be awarding ***
     c) the deadline

4) Hire an outside artist to draw an avatar for you. *****
You can check in
The Lair of Pretentious Artists forum, the trade channel, or ask people who already have custom avatars who did theirs. Each artist has their own way of doing things.
I recommend ironing out the following details beforehand:
     a) what you want the image to look like

     b) the fee
***
     c) when the art will be done
     d) if the artist will assist you again if the image is rejected
* Anything that has original art or animation will have an extremely difficult time getting through.

** You must choose a winner, even if there are no entries that you like or are even acceptable. You hold a contest at your own risk, not theirs.

*** You must pay the artist once you receive the art. You can not wait to see if it is accepted or rejected. You use an outside artist at your own risk, not theirs.

**** Once the artist has finished the image they are not required to assist you any further, even if the image is rejected. You use an outside artist at your own risk
