Print the following forms: Player Registration Form, Parent Code of Conduct and bring the appropriate registration fee (payable to PVYA basketball) and copy of birth certificate. Retain copies for your records. If you are interested in coaching, include the appropriate additional forms.

** Fees MUST be paid at registration. A COPY of the child�s birth certificate is MANDATORY at registration for new players ages 9 and older.** (No exceptions.)
IMPORTANT NOTES: No late registrations will be accepted. Students trying out for school
teams are asked to sign up for PVYA. Fees are refunded for players making the school teams.
Players who did not return uniforms from last year are ineligible.

COMMUNITY LEAGUE for Boys & Girls Ages 10-15
Travel league beginning in Nov., games are Dec. � March. Travel is required to the Lehigh Valley and surrounding areas. Practices and games are scheduled on various weeknights and weekends. Practices are held at PVE, PVI, MS, HS, 6-7:30pm or 7:30-9pm on weekdays, Saturdays anytime and Sundays after 12 noon. Average 6 home games (all weekends) and 6 away games (anytime). Cost is $80 per player
9- YEAR-OLD INTRAMURAL for Boys & Girls Age 9
PVYA is offering a 9-year-old intramural program. All games and practices will be held at Pleasant Valley Schools. No out-of-the-area traveling required. Teams will be gender specific and will play other PVYA teams. Help us start this program on the right foot by signing up your child today! Practices will be held weekday evenings (6-7:30pm) and weekends. Games will be held Saturdays (anytime) and Sundays (after 12 noon). COACHES ARE NEEDED. See the PVYA website for coaching forms, or ask at sign-ups for more information. Cost is $60 per player
YOUTH INSTRUCTIONAL for Boys & Girls Ages 7 & 8
8-10 week program (Saturday mornings), held at PVE, beginning January 10, 2009. Teaches basketball basics, and FUN! Program coordinators demonstrate skills and parent assistants practice skills with players. Parents must wait in the lobby during the session, but are invited in for scrimmages. Two sessions: 9am and 10:30 am. You will receive a postcard in December with your child�s session time. Cost is $35 per player
