Chapter 3: Substitute products

 

              It is thus necessary to remove all rich foods in protein

No meat, fish, egg, cheese, pastry making

and to replace them by special products called hydrolysats of proteins or preparations of amino acids more or less removed from their ph�nylanaline.

 

Currently, the choice of special food available in Belgium is very limited  ! Two main categories are distinguished  : on the one hand rich foods in proteins but low in ph�nylanaline, and on the other hand, products of replacement poor overall out of proteins, therefore in ph�nylanaline.

In the first category, we have in our country at our disposal several preparations of which 3 only are recorded and refunded. These three powders of amino acids that one can obtain easily are the phenyldon AM (Nutricia), Lofenalac (Mead Johnson) and the PKU   2, the PKU1, 1 mix and 3, (Milupa) are not recorded, but are compared to the others and consequently not refunded.

The phenyldon AM and the range of the PKU of Milupa are powders containing of the amino acids, the glucides, minerals and the vitamins. Lofenalac is a product which contains a hydrolysat proteins of which the quantity of ph�nylanaline is stable (77 Mg by 100 G of powder), of the glucides, greases and rock salt.

More recently appeared a series of products to the varied tastes, but they are still not refunded by the INAMI. Certain families depending on other systems of Social Security nevertheless had access to these new products.
All the preparations of amino acids and the protein hydrolysats have bad taste and release a bad smell.
The environment in which is placed the child is significant, because this one will enable him to form its taste, and,   in addition, to accept or refuse this food. It is necessary thus that the family makes a real effort not to influence the acceptance of the mode by the child. This one will be accustomed gradually to the product, as one is accustomed to other restrictions, such as for example removing sugar in his coffee.

Until now, industries could not improve savour of the product, because the metal taste which dominates is related to the method of manufacture of this foodstuff. The problem of the acceptability of the mode remains dominating. We hope to give you hereafter some ideas to mask the unpleasant taste and to thus allow the child to take his daily ration which will be increasing as it grows.
The mode of child PCU in practice.

        The establishment of a mode for a child PCU is based on two great principles  :

-    to bring to the child all the elements which it needs for his growth while deviating least possible from a normal feeding  ;

- to bring a correct quantity of ph�nylanaline and proteins corresponding to the individual needs of each child.

The calculation of the mode low in phenylalanine varies according to the weight, the age, the expenditure energy and blood rate's of each child. It will be composed  :

- natural foods which will meet its requirement out of phenylalanine.
- products without or low in ph�nylanalinee which will meet the requirements out of       proteins, heating and mineral necessary to ensure a normal growth.

Thus, the new-born baby will receive it several feeding-bottles per day. At the 3-5 month age, will be introduced the panades fruits initially, vegetables then. The only perceptible difference for a specialist will be the absence of meat added to the vegetable panades.

An example hereafter is better than a long explanation.

The preparation of the feeding-bottles is done without problem. The special product used generally during this period is Lofenalac or the PKU 1 mix, because their composition and the preparation is similar to the dried milk available for children not PCU. Lofenalac and the PKU 1 mix   dissolve better in tepid water (35�) and the feeding-bottles are preserved at 4� during 24h without problem. There is advantage to prepare in once the quantity necessary for all the day. This product of replacement contains an insufficient quantity of phenylalanine to meet the needs of the new-born baby. (7 Mg by mesurette)  ; it is necessary thus to add a small quantity of normal milk to balance child PCU. the experiment showed that the children of this age needed between 250 and 350 Mg for ph�nylanaline per day. For more facility it is dried milk which will be used, because the concentration in ph�nylanaline is standardized and contains according to various milks from 35 to 50 Mg by mesurette. This powder will be mixed in the whole of the feeding-bottles to avoid a too significant modification of the taste of the preparation between each meal. Lastly, the doctor will prescribe vitamins and a supplement of rock salt which remain necessary for a harmonious growth.

Example  : a mode for a one month old child.

Lofenalac              8 measurements.

Almiron m2           3 � measurements

Water                        to 600 ml.

Vitamins AD        6 drops.

Zymafluor              1 compressed.
