With 104,352 miles on my truck the block Heater went out, and it was mid January with temps in the single didgits at night. I order a new heater elements though Cummins of Cumberland for $47.43.
The kit shown above though Cummins inclued the element and new power cord.
The new heating element shown on the right
Step 1 is to drain the coolant system of the engine.
The heating element is located just behind and to the left of the oil filter
Disconnect the power cord from the element and use a 1 1/8 open wrench to back the element out of the block
Pull the old element out of the block and apply teflon tape or thread sealant to the new element threads and insert in the block cavity wall and tighten down with the 1 1/8 wrench
Old element with  104,xxx miles on truck and 3 1/2 years old
Take out the old power cord and route the new cord in and tighten in the connector to the element.
Refill the coolant system and check for leaks.
I removed the air filter and hose connecting to the turbo for installtion of new block heater
