As
a service to our community, Aiea Public library processes applications for
U.S. passports. If you need to
get a passport, here is some helpful information for you. First and foremost, during each shift that we process passports, we only accept 10-12 applications. We do NOT accept the passport mail in renewal form. If
you have a U.S. passport that was issued 15 years ago or less, and you were at
least 16 years of age, you can use a mail-in
passport renewal form. Anyone
that cannot use the mail-in passport renewal form (including children) must
apply in person and must bring the following items with them:
1.
DS-11 passport
application form
2.
Current picture ID
3.
2 identical passport
pictures. These would be color pictures
taken against a light background within the past 6 months.
Some examples of places that you can take passport pictures at are:
Sam’s Club, Costco, Sears, Longs, and Kinko’s.
There is also a One-hour Photo shop in Pearlridge Center near Macy’s
that takes passport pictures.
4.
Proof of citizenship.
This must be a certified copy of a US birth certificate or
naturalization certificate. An
old US passport may also be submitted as proof of citizenship.
Hospital birth certificates, and birth abstracts from Texas and
California are NOT acceptable. 5. Check or money order. If you submit your applications at Aiea Public Library, the fees are:
Children 13 and younger must be accompanied by BOTH parents with current picture ID. If one parent cannot make it, he or she must sign and notarize a consent form. In addition, if a child 13 or younger is applying for a new passport and has a previous passport, parents must also bring a copy of the birth certificate. Children 14-17 years of age must be accompanied by at least one parent with current picture ID. All children applying for passports MUST be present at the time of application. Please
allow up to 6 weeks to receive your passport.
You may also expedite processing for an ADDITIONAL $60 to receive your
passport within 2 weeks. Aiea Library's passport hours are:
