Management

Pain management is one of the highest priorities in the postoperative plan of care. Pain is expected and anticipated.

  • postsurgical pain is often severe for 48 hours, notable for 5 days, and progressively lessens and resolves in 2-3 weeks after surgery
  • most patients need round-the-clock analgesia for 2 to 5 days and then periodic analgesia is generally adequate
  • for severe pain after surgery, opioids, nonopioids, and local anesthetics are important pharmacologic agents

It is important that the patient understands the value of adequate analgesia and the negative impact of unrelieved pain. The patient must realize that using analgesics on a regular schedule (before pain gets too severe) is important because preventing significant pain is easier than capturing and controlling severe pain. Improved pain management equals better maintenance of comfort.
